IMPORTANT INFORMATION REGARDING COVID-19 VACCINE

MEDICAL EXEMPTION LETTERS:

There are three rare situations in which a patient would qualify for medical exemption from the COVID-19 vaccine:

1.     Severe allergic reaction (i.e., anaphylaxis) to a previous dose of a COVID-19 mRNA vaccine, as confirmed by an allergist

2.     An allergy to a component (PEG (Polyethylene glycol), Tromethamine and Polysorbate), of the COVID-19 vaccine, as confirmed by an allergist

3.     Myocarditis or pericarditis after a first dose of an mRNA vaccine – as a precautionary measure, the second dose of the series should be deferred in individuals who experience myocarditis or pericarditis following a first dose of an mRNA vaccine as confirmed by a physician, until more information is available.

***The physicians at Grandview will be writing COVID-19 vaccination medical exemption notes based on the criteria laid out by the College of Physicians and Surgeons of Ontario.  The CPSO advises that your physician is not able to provide a letter to exempt you from a COVID vaccine on the basis of any religious or personal beliefs, or for any medical condition not listed above.***

Memory Clinic

Grandview's Memory Clinic is a free service provided for our patients who feel they are having memory problems, have had memory issues and a family history of dementia or patients who have been seen by Dr Yang and are now being followed by their family Doctor.

We have 2 Memory Clinic Teams. Each team is comprised of 1 doctor, 1 nurse, 1 pharmacist and we share a Social Worker and a  Dementia Support Councellor from the Alzheimer Society. We hold our clinics once or twice a month.  A referral from your family doctor is needed in order to be assessed at the clinic.  Patients are not able to self refer.  Referrals are then reviewed and appointments are given according to urgency.

The wait time to be seen can range from 1-3 months generally.  Once an appointment is made the patient will receive a package in the mail. Included in the package is a blood work requisition which the patient must have completed at least 1 week prior to their appointment. Also included in the package is a questionnaire and short history taking page. These must be brought back with you on the day of your appointment.

Appointments take anywhere from 2-3 hours.  Patients MUST bring a family member or someone who knows them really well with them to the appointment. They must also bring all of their medications in the original packaging/bottles, and any eye glasses or hearing aids that they may wear.   As part of the assessment we will provide comprehensive history taking and cognitive testing which will include assessing the ability to drive.  This does not include a road test.  If it is deemed that the patient has a diagnosis of a memory related problem then we will see them again for follow up at our clinic.

Patients do not require more blood work for follow up visits unless specified.  If you need to speak with our team you may call 519-623-4200 option 6.
