At Grandview Medical Centre, we take pride in being one of Ontario’s pioneering Family Health Teams, bringing together a diverse group of healthcare professionals to deliver coordinated, patient-centered care. Our goal is to provide resources and support that enhance your health and well-being through the information and services available on this website.

Established as a Health Service Organization (HSO) in 1989 and transitioning to a Family Health Team in 2006, we are proud of our interdisciplinary approach. Our team includes physicians, nurse clinicians, registered nurses, registered dietitians, social workers, a health promoter, and clinical pharmacists—working together to offer comprehensive, seamless primary healthcare services.
